Academic Suspension Appeal Form

First Level of Appeal

If you are on academic suspension from Texas State and would like to appeal for reinstatement, you must complete the following form. Please also read the University policy on Academic Probation and Suspension in the undergraduate catalog at the following link: Academic Probation and Suspension Policies.

Final Level of Appeal

Students who are denied reinstatement by the College of Health Professions may appeal to the Texas State University Suspension Appeals Committee. If your appeal is denied, Dr. Erbin-Roesemann will provide information outlining the process for appealing to the Texas State University Appeals Committee.

Statement of Circumstances

Please submit a written document, addressed to Dr. Marla Erbin-Roesemann, explaining the extenuating circumstances that led to your academic difficulties. Be as thorough and detailed as possible. Proper spelling, punctuation, and grammar are expected. This explanation is your “letter” of appeal, so ensure it is well-written and adequately addresses your circumstances. In your letter, you should also include the answers to the following questions:

  • What steps have you taken to address these circumstances that lead to your academic difficulty?
  • If your reinstatement is not approved, what will you be doing during your time away from Texas State?

Supporting Documentation

Supporting documentation is required and should be from the semester(s) in which you had academic difficulty. Examples of documentation include:

  • Hospital bills
  • Obituary
  • Divorce decree
  • Medical or disability diagnosis
  • Work record
  • Police report
  • Court records
  • Transcript of successful coursework at another institution since your latest Texas State enrollment

Final Check List!

Your Suspension Appeal is not considered complete until you have secure transferred your letter to the Associate Dean, as well as your supporting documentation, and have submitted this form by clicking the submit button below (will not submit unless all mandatory fields have been filled out). 

Incomplete appeals or appeals submitted past the deadlines stated in your Suspension Letter will not be reviewed.
