Financial need will be the primary factor in determining eligibility for services. Potential clients agree to complete the Eligibility Intake Form so that financial need can be assessed. To be eligible for services, a potential client must have a household income less than 300% of the Federal Poverty Level AND must meet one of the following criteria: 

  • Be uninsured
  • Have no remaining physical therapy benefits for the year
  • Have a significantly high co-pay or deductible for physical therapy compared to disposable income

Preference for appointments will be given to those with a significant debilitating condition, and we are unable to treat patients under 18 years of age. 

Eligibility will be decided by our 5 referral sources listed below or faculty discretion. If you are interested in being seen in the Community Access Clinic, please receive a referral from one of the providers listed below. Once your referral has been received by the Community Access Clinic, you will be contacted by our scheduling team. The clinic often has a waiting list therefore it may be a couple weeks until you are contacted.
