This form will provide advisors with information that will assist them in preparing for your advising appointment. Your appointment will not be scheduled until this form is submitted and processed by our office.

Questionnaire Processing

After our office receives your New Student Information Questionnaire, an advisor will:

1. Update your major to the major you list on the form, 
2. Make any necessary updates to your degree audit, and
3. Identify any courses that need further evaluation to aid in the advising process. 

Note: An advisor may reach out to you with instructions on how to have specific courses evaluated for credit toward your desired major. 

Once your information has been updated and courses evaluated for degree applicability, our office will contact you to provide advising options.
